Responsibilities
ASEANA HEALTHCARE PTE LTD
is looking for aresponsible, organised and caring
Care Coordinator
to join our growinghealthcare team.
Key Responsibilities
Coordinate and manage
ICS referrals
from hospitals
Contact clients and families to arrange the start of care services
Coordinate
Nursing Aide/Caregiver assignments and schedules
Monitor daily attendance and ongoing client services
Arrange replacement staff when necessary
Communicate regularly with clients, families, hospitals and the ICS team
Monitor clients' service periods and upcoming discharge dates
Who We're Looking For
Diploma or relevant experience in
Healthcare, Nursing, Community Care, Social Services or Healthcare Administration
Experience in healthcare/home care coordination is an advantage
Good communication and interpersonal skills
Well-organised and able to manage multiple cases
Comfortable coordinating staff schedules and handling last-minute changes
Good computer and administrative skills
