Responsibilities
The job holder is responsible for performing care assessment, planning, and management for residents, according to established protocols and evidence-based practices, under appropriate supervision.
S/He will be expected to demonstrate competency in care coordination across the healthcare continuum and provides holistic care through effective collaboration with interdisciplinary teams and relevant community partners. S/He facilitates care transition and coordination for residents based on their individual needs and preferences to encourage self-management and continuity of care.
The Staff Nurse contributes to evidence-based practices, innovation, quality improvement and/or research projects.

As a Senior / Staff Nurse in Community Care, you will be required to perform nursing care and health coaching within the areas of:

Job Description
As a Senior / Staff Nurse in Community Care, you will be required to perform nursing care and health coaching within the areas of:
- Health Promotion & Illness Prevention through patient education
- Primary Care by performing basic assessment and health education to the general public
- Home Care services for patients transiting from hospital to home
- Any other community related duties will be assigned.
- Possess Diploma / Degree in Nursing and must be registered with Singapore Nursing Board
- Minimum 2 – 3 years of relevant work experience in Community Setting
- Able to work independently and as a team
- Strong organisational, coordination, interpersonal and communication skills
- Able to converse in local languages is preferred
