Specific Management of Care
Perform & coordinate general nursing care within the team on assigned patients of relatively lower acuity, or to support RN in-charge in Enhanced Care Area (ECA)
Systematically and efficiently collects relevant clinical and psychosocial data about on the assigned patients
Applying clinical reasoning and critical thinking, interpret patient information to identify the patients’ actual and potential health conditions, needs and risks, and make critical decisions about needed actions including use of escalation system
Set measurable and achievable short- and long-range goals with the patients, and plan evidence-based preventive measures and active interventions accordingly
Implement care plan and evaluate the effectiveness
Use effective communications to coordinate and evaluate nursing care
Prepare for timely discharge of the patients and ensure continuity of care both during hospitalization and after discharge to the community
Empower patients and care givers with standardized Patient/Family Education (PFE) (with or without guidance) to address the learning needs of the assigned patients and their families with general and basic care needs
Professional, Legal and Ethical Nursing Practice
Participate in the multi-disciplinary team’s plan and care of patients.
Document the nursing process objectively, accurately, timely and completely
Effectively communicates critical clinical situation using SBAR
Leadership and Nursing Management
Lead and guide junior members of the care team including Enrolled Nurses, student nurses and Healthcare Assistants, in the delivery of safe and effective patient care
Adhere to hospital guidelines and standards, and report any clinical error or near miss
Practise safety principles to maintain a safe environment for patients, staff and visitors
Educates patients and visitors on risk management; including early identification of high risk patients with accurate use of risk assessment tools
Manage material resources by minimizing wastage and proper use of equipment
Manage manpower within cubicles assigned by prioritizing the activities that contribute to desired patient outcomes
Professional Development
Actively seeks knowledge through participation in professional development activities and beyond prescribed education programmes
Pro-actively consults senior nurses when unsure of clinical practice or about professional development
Achieve 15 CPE points and at least 60 hours of continuing nursing education
General
Assume accountability for one’s own and delegated nursing care
Advocate for the rights and needs of patients
Protect patient privacy and confidentiality of patient records and other privileged communications
Consistently demonstrates evidence-based practice to affect best patient care
Knows how to find the best possible sources of evidence to apply in practice
Participate in until/hospital initiatives on quality improvements (e.g. risk management), and on productivity improvements
Perform any other duties / projects as assigned by supervisor
