Responsibilities for Social Work & Case Management Officer:

• Develop individualised care plans based on residents’ needs and conditions.
• Implement care plans and monitor residents’ progress and care outcomes.
• Develop and coordinate programmes and activities to support residents’ care and well-being needs.
• Prepare six-monthly care review reports and conduct monthly interviews for approximately 50 residents.

Qualifications for Social Work & Case Management Officer:

• Degree or Graduate Diploma in Social Work, Psychology or Counselling.
• Willing to adhere to established Standard Operating Procedures (SOPs) when carrying out duties.
• Willing to work rotating shifts according to the duty roster.
• Minimum 1 year of relevant work experience.
