Specific Management of Care
Perform & coordinate general nursing care within the team on assigned patients of relatively higher acute e.g. RN in-charge in Enhanced Care Area (ECA)
Systematically and efficiently collects relevant clinical and psychosocial data about on the assigned patients
Applying clinical reasoning and critical thinking, interpret patient information to identify the patients’ actual and potential health conditions, needs and risks, and make critical decisions about needed actions including use of escalation system.
Set measurable and achievable short- and long-range goals with the patients, and plan evidence-based preventive measures and active interventions accordingly
Implement care plan and evaluate the effectiveness
Use effective communications to coordinate and evaluate nursing care
Prepare for timely discharge of the patients and ensure continuity of care both during hospitalization and after discharge to the community
Independently conducts standardized Patient/Family Education (PFE) by co-managing the learning needs of the assigned patients and their families with complex care needs
Provides supports to junior nurses in PFE activities when required
Professional, Legal and Ethical Nursing Practice
Actively contribute to the multi-disciplinary team’s plan and care of the patients
Independently document the nursing process, and demonstrates critical thinking, advocacy for the patients, and awareness of medico-legal implications
Effectively communicates critical clinical situation using SBAR, demonstrating greater confidence and competence e.g. with precise assessment and recommendation
Leadership and Nursing Management
Lead and guide junior members of the care team including Enrolled Nurses, student nurses and Healthcare Assistants, in the delivery of safe and effective patient care
Professional Development
Manages own learning needs and improve clinical competency by contributing to unit-level professional development activities e.g. by sharing knowledge and reflection of learning
Assume the role of preceptor to help trainees and new/junior nurses cultivate evidence-based practice and gain clinical independence
Achieve 15 CPE points and at least 60 hours of continuing nursing education
General
Assume accountability for one’s own and delegated nursing care
Advocate for the rights and needs of patients
Protect patient privacy and confidentiality of patient records and other privileged communications
Consistently demonstrates evidence-based practice to affect best patient care
Knows how to find the best possible sources of evidence to apply in practice
Actively contribute to (e.g. undertake special assignment) in unit/hospital initiatives on quality improvements (e.g. risk management), and productivity improvements
Perform any other duties / projects as assigned by supervisor
