Responsibilities:
Conduct thorough health assessments, analyzing residents' health status and risk factors.

Collaborate with residents and relevant stakeholders to establish health goals using the S.M.A.R.T. framework.

Create customized health coaching plans that motivate residents towards achieving their wellness objectives.

Assist residents in overcoming challenges related to lifestyle and behavioral changes.

Monitor and document residents' progress and health outcomes to ensure effective care.

Requirements:
Diploma or Advanced Diploma in a healthcare-related discipline.

Strong ability to work independently as well as collaboratively within a team.

Excellent communication and engagement skills, particularly with elderly.

Demonstrated organizational, coordination, and interpersonal capabilities.

Proficient with computer skills and Microsoft Office applications.
