My client a non-profit Dementia Centre was established to support seniors living with dementia through holistic, person-centred care that enables them to remain active and engaged within the community. Working alongside a multidisciplinary team of therapists and care professionals, their nurses play a key role in enhancing the quality of life of our clients and supporting their caregivers.

Key Responsibilities

Clinical Care

• Perform nursing assessments and ongoing monitoring of clients attending the Dementia Day Care Centre.
• Monitor vital signs, cognitive status, behavioural changes and overall well-being of clients.
• Administer medications and ensure medication safety and compliance.
• Attend to basic nursing procedures and first aid requirements when required.
• Escalate clinical concerns promptly and coordinate follow-up care with external healthcare providers.

Dementia Care & Behavioural Support

• Support persons living with dementia using person-centred care approaches.
• Observe, identify and document behavioural and psychological symptoms associated with dementia.
• Implement strategies to manage challenging behaviours safely and compassionately.
• Promote clients' independence while ensuring safety and dignity.

Programme Participation

• Support and facilitate daily therapeutic, social, cognitive and recreational programmes.
• Encourage client engagement during activities designed to maintain cognitive and physical functioning.
• Collaborate closely with Occupational Therapists, Physiotherapists and Therapy Assistants to achieve care goals. 

Caregiver Engagement

• Build strong partnerships with family members and caregivers.
• Provide education and guidance on dementia care and daily management strategies.
• Communicate clients' progress, concerns and recommendations effectively.

Requirements

Essential

• Registered Nurse with valid Singapore Nursing Board (SNB) registration.
• Minimum 2 years of nursing experience.
• Strong communication and interpersonal skills.
• Passion for eldercare and community healthcare.

Preferred

• Experience in dementia care, geriatric nursing, community care, nursing homes, day rehabilitation centres or eldercare services.
• Certification or training in dementia care.
• Familiarity with person-centred care approaches.
• Experience working with multidisciplinary healthcare teams.
