Responsibilities
Job Responsibilities
A). Outreach
Promote youth mental health awareness through social media engagement, talks, workshops, roadshows and campaigns.
Customise and design suitable outreach events based on the needs of the youth and partners.
Create social media posts to reach out to youth in the community and bring awareness about CREST-Youth services.
Coordinate between different internal and external stakeholders to ensure smooth running of events such as volunteers, youth mentees, partners, SAMH youth centres and HQ.
Develop long-term partnership with organisations with common interest in supporting youth mental wellness, especially those residing in our region.
B). Case Management Support
Attend to phone enquiries from stakeholders or caregivers on referral process and criteria.
Support in intake interviews, triage and co-triage with team when deemed necessary.
Conduct regular check-ins, and emotional support to clients and caregivers to monitor progress and wellbeing.
Identify presenting/underlying issues to formulate a care plan to enhance or maintain and promote psychosocial functioning of clients.
Help coordinate case conferences and multi-disciplinary meetings.
C). Administrative and Documentation Tasks
Maintain accurate and timely documentation and data in accordance with case management protocols.
Support reporting officer in collating data and other statistics needed for programme.
Adhere to Programme’s key performance indicator (KPI).
D). Group Activities for In-Centre Youth
Design, plan and facilitate group activities for in-centre youth.
Work closely with volunteers and interns to conduct workshops for youth.
Collaborate with other youth centres to organise events for youth.
Accompany youth and their caregivers for external events to ensure their safety and well-being when needed.
Inform supervisor and reporting officer of any critical situations involving conflict, violence and other emergencies to seek support promptly.
E). Professional Development
Attend Individual and group supervisions, team meetings and training sessions to deepen knowledge in youth mental health.
Stay updated on best practices and resources relevant to adolescent development and mental wellbeing.

Other Duties
To assist Service Head and Team Lead to organise recreational activities for the Team when necessary.
To assist ad-hoc SAMH projects, community outreach programmes and events, ie World Mental Health Day, YOUth Alive!, etc. assigned by Head.
Any other duties as assigned only by Deputy Headand not any other staff.
Uphold SAMH professional standards and ethical guidelines.
Demonstrate a high level of trustworthiness, commitment to the organization and team’s mission and empowerment in daily tasks.
G. Operations at C’SAY CREST-Youth @North Centre
Ownership of centre space.
Maintenance of premises.
Qualifications & Experience
Minimum Diploma in Social Work, Counselling, Psychology or related discipline.
2 to 3 years of relevant work experience in Healthcare or Community Care is preferred.
Strong organisational and time management skills, with attention to detail.
Strong communication skills.
Must be proficient in MS Word, Excel and Powerpoint.
Ability to work independently and as part of a team.
Proactive, resourceful and able to multi-task.
