Responsibilities
Responsibilities:
Support the care team in coordinating and improving the patient’s care
Work closely with other nursing and allied team to provide advocate care options and services to the patients and their family members
Improve the patient’s care plan by understanding their needs
Involve in community outreach programmes
Requirements:
Degree in Social Work/Psychology
Working experience in healthcare or social service sector is an advantage
Ability to multi-task and work in a fast-paced and dynamic environment
