Full Time Senior-Social Worker (Community Care) Jobs, Salary up to SGD 4,000 in CELECTI PTE. LTD. Central Region (Singapore) - Maukerja

• Salary: Up to $6,500 (commensurate with experience)

• 5 Day Work Week

• Working location: South Central Singapore, with travel to community sites

• Permanent full-time

• Senior positions available depending on experience and qualifications

A leading provider of senior care and community-based healthcare services is currently seeking Social Workers and Senior Social Workers to join their growing team.

Responsibilities:

• Conduct psychosocial assessments and develop person-centred care plans for clients and caregivers

• Conduct assessments, care planning and interventions for seniors and caregivers

• Provide counselling, psychosocial support and advocate for clients’ needs

• Manage case referrals and participate in case meetings

• Coordinate and monitor services with internal teams and community partners

• Provide information on assistance schemes and care options to support informed decisions

• Support advance care planning (ACP) and end-of-life discussions where relevant

• Maintain accurate case records and progress reports in a timely manner

• Provide clinical supervision and guidance to junior staff (Senior Social Worker level)

Requirements:

• Min. Postgraduate Degree in Social Work recognised by the Singapore Association of Social Workers (SASW)

• Registered Social Worker (RSW) with SASW

• Min. 2 years of community or elder care social work experience, candidates with more experience may be considered for Senior positions

• Experience in palliative care settings will be an advantage

• Willing to travel for home visits and different sites where required (Travel allowance provided)
