Full Time Assistant Care Coordinator | Healthcare Jobs, Salary up to SGD 2,200 in PERSOL SINGAPORE PTE. LTD. Central Region (Singapore) - Maukerja

As an Assistant Care Coordinator, you will support patients receiving care at home by coordinating with healthcare teams, patients, and caregivers. You will help monitor patients' conditions, provide updates and education, and ensure they receive the care and support they need.

Key Responsibilities

• Conduct pre-admission screening for patients and caregivers.
• Coordinate with nurses and healthcare teams to support patient care at home.
• Monitor patients' condition and report any concerns or changes.
• Handle patient and caregiver enquiries and provide care plan updates.
• Develop and document care plans, patient progress, and treatment records.
• Attend care team meetings and contribute to patient care discussions.
• Arrange referrals and coordinate with community healthcare services.
• Monitor patients' vital signs through telehealth systems and escalate issues when needed.
• Provide basic patient education and health coaching on areas such as diet, exercise, and wellness.
• Ensure necessary equipment and supplies are available for patient care.
• Support quality improvement projects and perform other assigned duties.

Requirements

• Diploma in a healthcare-related field preferred.
• Basic computer proficiency.
• Comfortable to work shifts, including evenings, weekends, and public holidays.
• Able to work independently in a fast-paced environment.
• Customer-focused with strong interpersonal and communication skills.
• Good teamwork skills with some leadership potential.
• Proactive, responsible, and able to maintain professional integrity.
• Prior healthcare or hospital experience is an advantage.
