8-Month Contract

Responsibilities
• Conduct patient outreach and support enrolment into the programme
• Monitor daily dashboards and review patient responses from the patient-facing chatbot
• Route and escalate cases to the clinical team according to established workflows and protocols
• Follow up with non-responsive patients and guide them on using the chatbot
• Verify patient eligibility, enrolment status and dashboard records
• Maintain and update patient databases accurately and promptly
• Coordinate appointments, referrals, handovers and communication between patients and clinical teams
• Document patient interactions, outcomes and escalations across relevant IT systems
• Collate and validate data including enrolment, response rates, outreach attempts, escalations and patient
outcomes
• Analyse trends, care gaps and non-response patterns to improve patient engagement and operations
• Prepare reports and support audits, service evaluation and continuous improvement initiatives

Requirements
• Min. diploma or equivalent experience in Healthcare Administration, Care Coordination, Nursing Support, Population Health or related discipline
• Patient-facing healthcare, telehealth or chronic disease management experience advantageous
• Familiarity with electronic medical records and multidisciplinary healthcare environments beneficial
• Good patient communication skills via telephone and digital channels
