Responsibilities
Plan, coordinate, and monitor the health and well-being management of designated stakeholders.
Provide individualized, person-centred care, including routine blood glucose and blood pressure monitoring.
Support lifestyle and dietary management in accordance with individual health needs and appropriate care plans.
Provide necessary health and physical care to stakeholders during domestic and international travel, as required.
Monitor general well-being, maintain relevant health records, and communicate health-related observations to the appropriate healthcare professionals when necessary.
Support continuity of care by ensuring that relevant health information, monitoring requirements, and care arrangements are appropriately coordinated during travel.
