Job Description
Responsibilities
Provide Occupational Therapy services to patients in inpatient, outpatient and home-based settings.
Cover assigned wards and manage allocated patient caseloads.
Conduct initial assessments and regular reviews of patients' conditions and functional abilities.
Develop individualised rehabilitation goals and treatment plans.
Review and update rehabilitation plans weekly.
Conduct reassessments every six months for maintenance cases and every three months for active rehabilitation cases.
Monitor treatment progress and update clinical records.
Coordinate patient follow-ups and discharge planning.
Attend case conferences, team meetings and family meetings.
Work closely with Physiotherapists and other healthcare professionals to coordinate patient rehabilitation.
Educate patients, families and caregivers on rehabilitation activities and safe care practices.
Conduct caregiver training when required.
Ensure patient safety during therapy sessions and carry out appropriate risk assessments.
Ensure rehabilitation equipment is used safely and report any defects or incidents.
Maintain proper clinical documentation and handovers.
Follow hospital guidelines and professional standards.
