Kerja Sepenuh Masa Case Manager, Gaji tinggi MYR 3,500 di IHT Rehabilitation Centre Johor - Maukerja

Job Responsibilities

• Coordinate and manage cases from admission to discharge.
• Serve as the primary liaison between patients, families, healthcare professionals, and external stakeholders.
• Work closely with Case Managers of Pusat Rehab PERKESO (PRP) and Disability Management Officers from local PERKESO Office.
• Conduct patient follow-ups, home visits, and provide ongoing support to ensure rehabilitation goals are achieved.
• Monitor patient progress and facilitate timely interventions to support recovery and return-to-work outcomes.
• Maintain accurate case records, reports, and documentation in accordance with organizational requirements.
• Track, compile, and analyze patient-related data, including patient contacts, home visits, and rehabilitation outcomes for management reporting.
• Provide information, guidance, and support to patients and caregivers throughout the rehabilitation journey.
• Collaborate with multidisciplinary teams to ensure coordinated, patient-centered care and service delivery.
• Participate in quality improvement initiatives and ensure compliance with relevant policies and procedures.

Job Requirements;

• Diploma/Degree in Nursing, Medical Assistant, Social Work or equivalent.
• Minimum of three (3) years' customer service experience, preferably in human services, possess demonstrated commitment to customer service.
• Professional or personal experience supporting individuals with disabilities, or challenging behaviors highly preferred
• Must be willing to travel and possess own transport.

Pay: RM2,800.00 - RM3,500.00 per month
