Responsibilities:

· Plan and conduct meaningful activities with persons living with dementia at home.

· Curate activities based on the abilities/needs of the person living with dementia.

· Complete service record for each session.

· Provide regular feedback and updates to caseworkers.

· Attend regular case conferences / sharing group sessions.

Requirements:

· Comfortable to travel independently to client’s home for session.

· Training will be provided. Minimum hours to commit after training.

· Hourly rated casual employment with transport allowance.
