Job Summary

Metta Home for the Disabled provides long-term residential care for adults with intellectual and multiple disabilities. As a Social Work Assistant, you will support Social Workers and the multidisciplinary team to enhance residents’ well-being, coordinate programmes, and provide administrative support for smooth social work service delivery.

Responsibilities

1. Case Management & Client Care

• Conduct client assessments and manage admissions, care planning, reviews, and discharges.
• Monitor clients' needs and coordinate care with the multidisciplinary team.
• Conduct home or hospital visits when required.

2. Caregiver Support

• Provide counselling, emotional support, and caregiver education.
• Assist with Means Testing, financial assistance applications, and access to community resources.

3. Multidisciplinary Collaboration

• Work closely with healthcare professionals and participate in case discussions and service planning to ensure holistic care.

4. Documentation & Reporting

• Maintain accurate client records, reports, and database updates.
• Prepare and submit subsidy claims, monthly reports, and required statistics.
• Ensure compliance with organizational policies and regulatory requirements.

5. Other Duties

• Supervise and guide Social Work Administration staff.
• Support audits, organizational projects, and events.
• Perform other duties assigned by management.
