This role involves conducting comprehensive care assessments and engage residents in health goal-setting. Implement lifestyle interventions to reduce health risks, support chronic disease self-management, and improve quality of life through sustainable behaviour change. Collaborate with the Community Health Team to ensure holistic care planning and effective service delivery.

Job Responsibilities

• Assess residents' health status and risk factors through subjective and objective evaluation.
• Facilitate collaborative goal-setting with residents and stakeholders using S.M.A.R.T. framework.
• Design tailored health coaching plans to guide residents towards wellness goals.
• Support residents in addressing lifestyle and behavioural change challenges.
• Track and document resident progress and health outcomes

Job Requirements

• Diploma/Advanced Diploma in healthcare-related field.
• Ability to work independently and as part of a team.
• Strong communication and social engagement skills, particularly with older adults.
• Excellent organisational, coordination, and interpersonal abilities.
• Proficiency in computer skills and Microsoft Office applications.
• Available to work weekends on a rotational schedule to support operations.
• Ability to travel to various work locations as required.
