Job Role

• Assist in running of targeted outreach and cognitive stimulation activities
• Support initial assessment of walk-ins & referral cases, follow-up with intervention plans for clients as per the prescribed care plan and under guidance from the Team Lead or Assistant Team Lead (in Team Lead's absence)
• Support in monitoring the Clients’ conditions and progress, and escalate to Team Lead or Assistant Team Lead

Requirement

• Diploma in relevant field (e.g. social work, psychology, counselling)
• Basic computer skills (MS Office – Excel, Outlook, Powerpoint and Word)
• Read & write emails

Work Experience

• Has minimum of 2 years of healthcare/ community care experience
