• Coordinate and support the planning, implementation, and evaluation of community care programmes for seniors and caregivers.
• Engage clients, caregivers, volunteers, and community partners to facilitate programme participation and service coordination.
• Monitor programme outcomes through data collection, reporting, and continuous quality improvement initiatives.
• Conduct ground visits and collaborate with internal and external stakeholders to enhance programme delivery and client experience.
• Support volunteer recruitment, onboarding, training, and coordination to ensure smooth programme operations.
• Manage programme administration, procurement, documentation, audits, grant support, and operational reporting.

Requirement

• Diploma/Degree in Healthcare, Social Work, Psychology, Public Health, Business, or related disciplines.
• Preferably 2 years of experience in healthcare or community care with strong stakeholder engagement skills.
• Proficient in Google Workspace and Microsoft Office applications.
