1. JOB DESCRIPTION
• To support and maximize the psychosocial functioning of dialysis patients and their families at
NKF. ESRD is a debilitating disease. The dialysis treatment derailed and altered the life course
of the patients and their families. It impacted them negatively, financially, psychologically, and
socially due to end-stage renal disease and the complexity of the treatment regimen.
• The NKF welfare services are provided to improve their interacting financial, physical,
psychological, and social conditions, which can impact outcomes, including a poorer quality of
life and uncertainty in the life course.
• To be in charge of and responsible for the team of Welfare Officers and Welfare Assistants in
implementing welfare-related programmes and activities within the organization to improve the
dialysis patients’ well-being.
2. KEY RESPONSIBILITIES
2.1 Patient Management:
i. Supervision and Support
a. Provide supervision and support in areas of patient management in registration,
engaging, interviewing, data collection, assessing, conceptualizing, planning,
intervening, and monitoring for the best patient outcomes.
ii. Standards in Processes, Procedures, and Practices – are maintained and conducted promptly in
the Welfare programmes and service delivery:
a. NKF HD Programme
i. Perform the social and financial assessment of new applications for admission
into the NKF HD programme,
b. MOH HD Subsidy application
i. Liaison with DCs and patients on the applications, including the renewal of MOH
HD subsidy
ii. Applications conform to the MOH standards for submission
iii. Monthly reporting to MOH on the list of patients receiving the MOH HD Subsidy
c. NKF Subsidies
i. The processes and SOPs for NKF subsidies are maintained
ii. Ensure delivery of the NKF HD subsidies with timely validated evaluative reports
iii. The subsidies budget expenditures are monitoredd. NKF Portable subsidies on NKF PD Subsidy, NKF Kidney Transplant Subsidy, NKF Rare
Kidney Disease Subsidy, and NKF Paediatric Vascular Access Subsidy.
i. Perform the social and financial assessment on the applications (new and
continual) for the subsidy with timely, evaluative, validated reports
ii. The subsidy budget expenditures are monitored
iii. Casework and Counseling
a. To deal with physical, emotional, social, and financial problems resulting from kidney
disease and dialysis by patients and their caregivers
b. To support patients in accepting, adjusting, and coping with their life on dialysis
c. To help improve their physical health and mental health outcomes
d. To motivate physically fit and young patients to Return to Work and vocational
rehabilitation
e. To provide Helpline support and counsel received (telephone, e-mails, etc) from the
general public in connection with NKF welfare services and other matters on
psychosocial aspects of kidney failure and treatment.
iv. Yearly Patient Review
a. To visit the patients at the dialysis centres at least once a year and also when the need
arises for review and provide counsel on psychosocial issues
b. The reviews are conducted on psychosocial issues and update the patient’s socio
economic status
c. Conduct HrQOL measurement to assess, address, research, and intervene to improve
outcomes
v. Networking
a. Network with relevant government and non-government organisations to promote and
assist patients with financial subsidies and other support
vi. Patient Selection & Welfare Committee
a. Organise the bi-monthly PSWC meetings
b. Seek formal approval of new applicants into NKF DCs
c. Present the Welfare Manager report on admission, subsidies, activities, and budget
utilised
d. Discuss and conclude on issues related to patient welfare
e. Prepare and seek Annual Welfare Budget approval
f. Decision-making on NKF welfare matters, policy, procedures, programmes and activities
2.2 Welfare Activities
vii. Family Day
a. Organise by the Dialysis Centres; raise the PR for the centres abiding by financial
procedures and processes
b. Monitor budget allocation
viii.NKF Patient & Caregiver Support Community
a. Facilitate the participation of NKF patients and caregivers to support (informal) one
another via NKF social media access
ix. Patient Forum
a. Plan and organize the Annual Patient Forum
2.3 Administrative Duties in the Welfare Department
Patient-Related:
a. Prepare the Welfare Budget (Patient & Staff) for approval by Management and PSWC
b. Monitor Welfare Budget and Subsidies, abiding by financial standards and procedures
c. Provide timely reports and updates to the Management
d. Prepare the annual report of the welfare department,
e. Liaise with Centre Managers/ Staff Nurses in charge of all NKF centres on patient
welfare matters
f. Other relevant duties in connection with patient welfare
Staff-Related:
g. Supervise staff to ensure smooth flow of work in the Department
h. Authorize staff leave, travel, claims, etc
i. Perform other duties as may be directed by the Head of Medical, Chief Executive
Officer, or Board of Directors.

5 POSITION SPECIFICATIONS
5.1
Qualifications
:
Basic Degree/ Diploma etc)
Basic degree in Social Work/Counseling
Be able to hold the Social Work Practising
Certificate, when the Social Work Profession Act
2026 is enforced.
Holder of a Counseling Practising Certificate.
