Job Description:
Responsibilities:
Provide quality nursing care and emotional support to ensure patient safety, comfort, and well-being.
Assess patients’ conditions, monitor vital signs, and administer medications and treatments accurately.
Develop and implement individualized care plans in collaboration with the healthcare team.
Conduct home or hospital visits and manage home care cases.
Support nursing cases and undertake respite assignments when required.
Guide care staff in managing medical issues and emergencies.
Communicate clearly with patients’ families on care updates and progress.
Coordinate care with internal teams and community partners.
Ensure compliance with MOH documentation and audit requirements.
Assist with complaint handling, crisis management, and other duties as assigned
Requirements:
Minimum Nitec/ Diploma/ Bachelor’s Degree in Nursing.
Advanced Diploma in Gerontology or Palliative Care is an advantage.
Must be an Enrolled Nurse or Registered Nurse with the Singapore Nursing Board (SNB) and possess a clean disciplinary record. (Nurses on conditional licenses are not eligible for this role.)
Minimum 2 years of clinical experience.
Proficient in Microsoft Office and comfortable using IT tools in daily work.
Ability to converse in local dialects will be an added advantage.
Willingness to perform occasional shift work, particularly to support palliative care patients.
