We are hiring under Recruit Express headcount and you will be deployed to:
Location: North / Northeast
Salary: $2500 - $3300
Duration: 1 Year
Working Hours: 5 Days / Office Hours
Job Responsibilities
Conduct health and lifestyle assessments for residents
Work with residents to set achievable health and wellness goals
Develop personalised health coaching plans
Guide residents on healthy lifestyle and behaviour changes
Support residents with chronic disease self-management
Monitor and document residents' progress and health outcomes
Work closely with the Community Health Team to coordinate care
Requirements
Minimum Diploma / Advanced Diploma in a healthcare-related field
Good communication and interpersonal skills
Able to work independently and within a team
Good organisational and coordination skills
Proficient in Microsoft Office
