Job Responsibilities

• Assess patients' care and discharge needs.

• Coordinate care plans with the multidisciplinary team.

• Facilitate discharge planning and referrals to community services.

• Educate and support patients and caregivers.

• Ensure continuity of care and smooth transition across healthcare settings.

• Maintain accurate documentation of patient care plans.
  

Job Requirement:

• Possess a Advanced/Higher/Graduate Diploma, Bachelor's Degree, Post Graduate Diploma, Professional Degree, Nursing or equivalent

• Minimum 5 years’ experience in acute care

• Demonstrated leadership and care coordination capabilities.

• Strong stakeholder management and interdisciplinary collaboration skills.

• Experience in quality improvement projects and mentoring junior staff will be an advantage.
