We're building a preventive and chronic care support programme for adults managing blood sugar, blood pressure, and cholesterol. As Health Coach / Counselor, you'll be the member's trusted point of contact — understanding their lifestyle, goals, motivation, and barriers across diet, exercise, sleep, work, and daily routines. You'll assemble each member's action plan with input from adjacent service providers like dietitians and fitness coaches, then run regular check-ins to assess adherence and make practical adjustments.

This is a coaching role, not a clinical one. You won't diagnose, prescribe, or change the doctor's clinical direction — your focus is motivation, behavior change, and accountability, working from a plan the doctor has already signed off.

What they do

• The member’s trusted point of contact.
• Understands the member’s lifestyle, goals, motivation, and barriers across diet, exercise, sleep, work, and daily routines.
• Assembles the action plan (AP) with input from relevant adjacent service providers (i.e., dietitians, fitness coach, etc.)
• Conducts regular check-ins to assess adherence and make necessary practical adjustments to the AP.

Who they are

• A counselor- or coach-type professional with strong listening and relationship-building skills.
• Experienced in behavior change, motivation, and accountability.
• Comfortable discussing daily habits, including food, exercise, sleep, work, stress, smoking, and routines.
• Engaged as a part-time/freelance consultant on a contract basis.
• Does not diagnose, prescribe, or change the doctor’s clinical direction.
• Based in Singapore
• Proficient in English
