Job Description:

• Assist and providing of support in monitoring clients' wellbeing through regular check-ins and directing clients to appropriate community resources
• Coordinating health and social care administration with various stakeholders.
• Maintaining of the updated documentation of care plans
• Encouraging and empowering clients towards healthy lifestyle changes
• Escalating complex cases to senior coordinators when required.
• Ensure consistent service standards, execute daily operational workflows and maintain confidentiality whilst safeguarding client data.
• Achieve service levels as determined and maintain low abandoned call rates through efficient call handling.
• Support community outreach and engagement efforts as required.
• Additional duties may be assigned by your supervisor as required.

Job Requirement:

• Min GCE N Level and Above.
• Min 4 years working experience contact centre or eldercare experience
• Required to work 12 hours rotating shift
