Job Description:
Assist and providing of support in monitoring clients' wellbeing through regular check-ins and directing clients to appropriate community resources
Coordinating health and social care administration with various stakeholders.
Maintaining of the updated documentation of care plans
Encouraging and empowering clients towards healthy lifestyle changes
Escalating complex cases to senior coordinators when required.
Ensure consistent service standards, execute daily operational workflows and maintain confidentiality whilst safeguarding client data.
Achieve service levels as determined and maintain low abandoned call rates through efficient call handling.
Support community outreach and engagement efforts as required.
Additional duties may be assigned by your supervisor as required.
Job Requirement:
Min GCE N Level and Above.
Min 4 years working experience contact centre or eldercare experience
Required to work 12 hours rotating shift
