Job Overview

We are seeking a dedicated Medical Social Worker to provide psychosocial support, counselling and case management for elderly residents and their families. You will conduct social assessments, facilitate Advance Care Planning (ACP), coordinate community resources and collaborate with multidisciplinary healthcare teams to enhance residents' well-being and support their care journey.

Key Responsibilities

• Conduct intake screenings and comprehensive psychosocial assessments to identify residents' social, emotional, financial and caregiving needs.

• Provide counselling, emotional support and crisis intervention to residents and families, including family mediation and end-of-life support.

• Conduct InterRAI assessments, develop intervention plans and maintain accurate case documentation.

• Facilitate Advance Care Planning (ACP) discussions, document residents' care preferences and promote ACP awareness.

• Coordinate referrals to community resources, social services, healthcare providers and government agencies.

• Collaborate with multidisciplinary teams on care planning, discharge planning and care transitions, while supporting departmental projects and service development initiatives.

Job Requirements

• Degree in Social Work (BA or Bachelor's Degree) or Graduate Diploma in Social Work.

• 3–5 years of relevant experience, preferably in a hospital or community hospital setting.

• Experience working with older adults and individuals with dementia.

• Registered Social Worker with the Social Work Accreditation and Advisory Board (SWAAB).

• Advanced Care Planning Facilitator certification is preferred.
