Kerja Sepenuh Masa Executive, Case Management (Part-time), Gaji tinggi SGD 3,400 di Villa Francis Home For The Aged - Maukerja

Duties:

• Provide administrative and case coordination support to the Social Work & Case Management team.
• Coordinate resident admissions, referrals and service arrangements with internal and external stakeholders.
• Liaise with residents, family members, hospitals, community partners and service providers to facilitate care support and service follow-up.
• Support care coordination activities, referral screening, reporting requirements and service transitions to ensure continuity of care.
• Assist residents and families with applications for financial assistance schemes.
• Prepare, submit and track referrals, applications and supporting documentation.
• Monitor case-related appointments, referrals and follow-up actions to ensure timely completion.
• Maintain accurate, timely and confidential records, reports and case documentation.
• Participate in multidisciplinary meetings and support departmental programmes and initiatives.

Requirements:

• Diploma or Degree in Psychology, Counselling, Social Sciences, Nursing or a related discipline.
• at least 3 years of relevant experience in healthcare, community care, social services, case management or care coordination preferred.
• Good interpersonal and communication skills.
• Strong organisational and coordination abilities.
• Able to work effectively in a multidisciplinary team environment.
• Compassionate, service-oriented and committed to supporting seniors and their families.
• Part-time: 4 days per week, office hours.

Pay: $3,000.00 - $3,400.00 per month
