Are you a qualified Occupational Therapist passionate about helping patients regain independence and improve their quality of life? Join our rehabilitation team to provide quality care across inpatient, outpatient and home-based settings.
Key Responsibilities
Provide Occupational Therapy services and manage assigned patient caseloads.
Conduct initial assessments and regular reassessments to monitor rehabilitation progress.
Develop individualised rehabilitation goals and treatment plans.
Implement evidence-based interventions and evaluate treatment outcomes.
Coordinate patient follow-ups, rehabilitation case management and discharge planning.
Collaborate with Physiotherapists and multidisciplinary healthcare professionals.
Participate in case conferences, team discussions and family meetings.
Provide patient education and caregiver training to support rehabilitation at home.
Maintain accurate clinical documentation and ensure proper handovers.
Ensure patient safety during rehabilitation sessions and report equipment defects or incidents.
Follow hospital clinical guidelines and professional standards.
