Join Tzu Chi Singapore's Active Ageing Centres and Community Support Centres to support the psycho-social well-being of seniors, young adults, and youths in the community. You will work closely with Social Workers and multidisciplinary teams to provide casework support, care coordination, outreach, and social intervention services that promote active ageing, social connectedness, and early identification of vulnerable individuals and families.

Job Descriptions:

1. Casework & Care Coordination

• Conduct intake and needs assessments for clients referred to the centre.
• Develop and implement care plans under supervision of Social Workers.
• Provide emotional support and basic counselling.
• Coordinate referrals and resource linkages with healthcare and social service partners.
• Monitor client progress and maintain case records.

2. Community Outreach & Engagement

• Conduct outreach activities and home visits to identify vulnerable individuals.
• Support onboarding of new participants into centre programmes.
• Connect clients to preventive health, wellness, and learning initiatives.

3. Group Work & Programme Support

• Assist in planning and facilitating support, therapeutic, and educational group programmes.
• Promote social inclusion, mental wellness, and intergenerational engagement.
• Work with volunteers and community partners to deliver meaningful activities.

4. Documentation & Reporting

• Maintain accurate case documentation in accordance with agency and sector requirements.
• Support data collection, programme evaluation, and reporting activities.

5. Professional Development

• Participate in case discussions, supervision, and multidisciplinary meetings.
• Attend training to strengthen competencies in social work, case management, and gerontology.

Requirements

• Passion for social service and community work.
• Strong communication and interpersonal skills.
• Able to work independently and in teams.
• Eligible for admission to a SUSS Social Work programme.
• Willing to undertake part-time studies while employed.
